How do I fill out the EDT form when signing up for CabMD?
When filling out the form listed here, you may wonder which options you'll need to check on the form.
Please select the options:
- Direct Dial (for Connectivity Method)
- No (for Network Provider)
- Billing Agent (under Registration)
