When you want to keep track of personal third party invoices, for services that are not a part of OHIP, you can use the Non-OHIP claim page to create these invoices.
When creating these claims, you can use a custom service code to label these invoices. DO NOT use any OHIP service codes to do this. Create your OWN 5 digit service code (for example ABC01, INS01, FORM1) that doesn't follow the OHIP format (Letter # # # Letter) format.
You can then create your own description for the service code and your own fee for that service. You would fill out the rest of the "To" and "From" sections as you normally would, and you can print off your own custom Non-OHIP invoices.
